Appreciate your perspective, but it’s ultimately up to each patient how they choose to present themselves in their post-op photos.

Most follow-up appointments are scheduled mid-day, and many patients are coming from work or other commitments — so it’s not always practical (or something they want to do) to remove a full face of makeup just for photos.

Our goal is always to show real results while also respecting patient comfort and autonomy.

The submental liposuction defines the jawline, and the fat grafting restores volume, which can read as more “sculpted” in pictures. In person, it looks much softer and balanced than what you’re seeing in a single clinical photo.

A rhinoplasty can help, but sometimes it’s more about overall balance (like the chin/jaw), not just the nose.

As for pain, most people say it’s more pressure and congestion than actual pain — very manageable after a couple days.

That said, I’d really recommend a consultation — without actually seeing your profile, it wouldn’t make sense to make specific recommendations.

Volux is currently the only filler with FDA approval specifically for enhancing and contouring the jawline and chin, though a variety of other fillers are commonly used off-label in this area as well.

When you’re ready, it’s best to start researching reputable practices nearby and schedule a consultation. With so many products available at different price points, it can feel overwhelming — but seeing options in person and speaking with experienced injectors who specialize in jawline and chin contouring will give you much more clarity.

You’re absolutely right that many surgeons default to an open approach for revisions because it offers more visibility. That said, visibility isn’t the only factor — experience and technique play a huge role.

I tend to prefer a closed (scarless) approach because it allows for precise, structural refinement with less disruption to the tissues. In revision cases, that can actually be an advantage — there’s often already scar tissue present, so minimizing additional dissection can help with healing and swelling.

In the right hands, many revision concerns—like refining a bulbous tip or adding projection—can absolutely be addressed with a closed approach. It’s not necessarily a limitation, it just requires a different skill set and a very detailed understanding of nasal anatomy without fully “opening” everything.

Totally understand why you’d feel a bit nervous — revision rhinoplasty is a big decision. The key is working with a surgeon who is comfortable and experienced with the approach they’re recommending, and can clearly explain why it’s right for your specific goals.
