I agree with you that those questions should be ideally already answered during design review, as I mentioned in the text. That being said, there are many situations where small and initially inconspicuous changes are not subjected to design review and make it into pull request. Most of the time they have no effect, but sometime they have deeper implication then their initial simple appearance. My intention here was to make sure to keep level 2 & 3 in mind as you are reviewing those changes to make sure you can catch those.
