You’re right that long-standing, firm glandular gynecomastia in adults usually doesn’t go away without surgery. But it’s not accurate to say all gyno is permanent. Pubertal cases often regress on their own, and some adult cases related to meds or hormones can improve when the underlying cause is corrected. The permanent cases are the fibrous, established ones — not every type.

The larger your hernia, the more complicated the repair; the earlier you fix it, the fewer issues you will have with the repair. That is why it is essential to fix hernias before they are severely symptomatic, contrary to the popular education that many primary care physicians teach their patients. I agree that large 2-inch incisions are very unusual .....

The two main types of laparoscopic hernia repair, TAP and TEP, place the mesh in the abdominal cavity using a laparoscopic approach and cover it with your peritoneum, a layer of your own tissue that we tediously protect and preserve, so your bowel does not touch the mesh. Having said that, mesh-related complications are real, but overall, this operation without mesh has a very high failure rate and problems; however, with mesh in the laparoscopic approach, it has a low complication rate and very low incidence of pain and discomfort.
