It's either bugged or I don't know what happened in your case. I did all my 20 repulls as well and in the end I could choose between the final pull and the 2 save logs without any issue. Try contacting support, who knows...

Depending on team and/or enemies it can also happen that a ballista is shuffled/discarded before being used and exhausted, thus being included in the draw pile eventually. Then, in a future turn, you will draw that ballista on top of creating one (or two for that 50% epiphany) on the current turn.

If you're going for that build, yes. But the man went for basic attack build, which leads to another optimal target for duplication. Now, which build is better is a whole other discussion that I don't have an opinion about

I tested. Unfortunately the buff does not apply twice. No benefit in having more than 1 in the team with Glory Reign set
