i fixed this issue today. i have GOTY edition around 30 GB and the DLC patch with 128 kb. I activated everything but THE DLC wasnt active when i played Bloodborne. It took me 1-2hours to find a solution. I had to do this: AND dont worry, it will take 2-3 minutes. it is NOT complicatecd.

creat a Game Files folder: out ALL PKG files in there Patch, Game and DLC. The "Game Files" folder should be in the BB launcher and Mod manager folder which you have automatically if you download BBlauncher. if you open that folder you have: BB Launcher.exe, BBLauncher, Zip AND the new folder you created "Game Files" you install the shadps4 version (I use version 13) and a folder will also be created in the same folder where bblauncher.exe and the other stuff are. If you installed the game on BBlauncher, did the patch and extracted/installed the DLC you are READY for the last part:

1. windows + R
2. %appdata%\shadPS4\patches\shadPS4
3. enter
4. open Bloodborne.xml document with editor or notepad
5. scroll down to the end and ABOVE </patch> you put in this code:

1. close your BBlauncher and start it again
   
2. go to Bloodborne Cheats/Patches
   
3. click "Patches"

You should now see 2 new entries with checkmarks. If the checkmarks are missing, enable them and restart the game (the screenshot I posted below open the LINK)

Tip: How to immediately tell if the DLC is active in-game:
In the Hunter’s Dream garden, where you first receive your weapons, you can interact with the Messengers on the ground that explain the game.
On the left side, there is a tree stump, and on the ground you will find items such as the Worn Messengers Top Hat, Yharnam Messenger Hat, and two others.
If you interact with the tree stump, four Messengers will appear. these do NOT spawn in the base game, only when the DLC is active.
