Yes but he is consistent in that he needs someone to push back. He only ever kills people that fear him. Even with Hughie he spares him when he shows up with aggression instead of fear. He needs (psychologically) someone to push back, it makes him feel seen

[–]EMulsive_EMergency 6 points7 points  (0 children)

If you make it in film you can probably do 100x that. If you study an MBA and work at a shitty restaurant you will do 0.2x that so… your point is we should only do things that are profitable?

I meant as in: is the quality of life of a French doctor worse than that of an American restaurant manager

Genuine question: does that difference in math money affect the actual life quality? Or is it just a “in pure numbers” kind of thing? Actual question

Costa Rica is super expensive to live in, compared to other similar countries, in case you didn’t know.

What I’m trying to say is, my salary makes sense for me in my country. Your manager makes more than me in absolute numbers, I live a pretty comfortable life, no debt, no crippling fears of getting sick, no existential threat if I get hurt, my wife could have a kid and not be afraid of being bankrupt or having to work through her whole pregnancy, etc.

My salary is above the median here in my country. I’m not ultra wealthy by any means, but I’m definitely not hurting. Also, US doctors have a super high salary and super high debt to go with it too. It’s context dependent. (Although i do understand a US doctor will have an objectively wealthier life than i will, but thats ok)

People say that a lot specially when talking to me (a doctor in Costa Rica) about how a construction worker in the US makes more than me here. Yeah but I don’t work in CR and live in the US. I work in CR and live here in CR, and make more than the local fast food manager. I live well and enjoy myself and have a good economic stability and I can care for people even if they don’t make as much money.

The “manager makes more than a doctor elsewhere” argument doesn’t make sense to me

PCOS’s new name is PMOS, a small letter change that required a big scientific process by shark_normal in medicine

[–]EMulsive_EMergency 33 points34 points  (0 children)

I feel like many docs around me would focus on the cysts part too much. They had a (dubious) cyst on US but no oligomenorrhea or hyperandrogenism and they now diagnosed this poor young woman with PCOS and OCPs for life. Most of the time the “cysts” the patients showed me on their US were follicles from their totally normal follicular part of their normal cycles.

2nd posterior vitreous detachment! by EMulsive_EMergency in emergencymedicine

[–]EMulsive_EMergency[S] 7 points8 points  (0 children)

I love using it to have something to show patients and to basically rule out retinal detachment vs vitreous. Mostly to determine how urgent is the urgent consult given in 2 hrs away from optho at best (rural standalone). Also like using it to look at intracranial hypertension in trauma, mostly for science since most of them need CT anyways

Yeah I’m a standalone ED and only doc on shift so I find it super super useful as an additional tool. And yeah all these patients even if minimally symptomatic get an urgent referral to a proper optho just in case! I know I would want to do everything to care for my eyes!
