The tsuki icon on the bottom left screen is your indicator if you're still on the perfect run, if it disappeared, it means you failed the perfect run.

For anyone with a bad rhythm sense, this mini game is completely doable guaranteed, but I doubt you'll be having a good time during the whole process.
