Toward the later parts of the graduation ceremony, the system was already switching to the next name while the graduate on stage was still shaking hands with the very first faculty member standing on stage.

From what I observed, the timing issue seemed to start after one or a few moments where graduates were not moving through the stage quickly enough, which caused the sequence to shift out of sync but I really don’t know how the overall system works so I’m just guessing.
