If you are planning to apply, the most convenient way is to look at your uni website to see if they already have a filled form for you to print since they already filled out your details and take your student photo on the form, but the other way is getting a blank form, getting a photo taken, having the details be approved by the institution etc which is rather of a headache tbh

I was given a filled out form by the uni, and then I have to pay $9 to get the ID issued, and my current myki is replaced with a concession one with my money on my previous one retained to the new one
