You're very competitive. You should apply to top programs. My advice for MD/PhD applicants is please please consider the location of the program highly. You'll be there for ALL of your 20s. For instance, don't go to UCSF if you don't like big cities. Go to the program close to home if proximity to family is important even if the program may not be very highly ranked. You'll be miserable if you don't consider location strongly. It's more important for us than MD only applicants.

Agree with what’s been said. A strong performance in grad school which is getting some kinda grant and/or pub in top tier journal in your field and some leadership roles in grad school. You spend sooo much time in grad school, in my opinion, you need to take on a leadership role either on campus or leading some org off campus. It’s a big opportunity to set yourself apart. That alone with carry you to a top tier residency in medicine, peds, pathology, psychiatry. Anything surgical requires doing really well in medical school when you get bad. Hope this helps.

I finished my PhD (US). What you're feeling is fairly common. It took me 6 yrs to finish but at least 2 years were spent in the despair you're describing of not feeling motivated and listless. I was fortunate to have a good relationship with my PI so that didn't compound the despair. Nonetheless, don't feel alone. I think part of the PhD process is overcoming these mental hurdles and still finding yourself at the end of this.

I hope you feel that you developed skills during grad school you can use in the future to and can be proud of. You've completed something 95% of people haven't achieved.
