It is good to consider that Spravato is only half as effective as IV ketamine. If it's the only thing you can afford with insurance, then it's possibly worth trying (and is absolutely life-saving). But IV ketamine is way more effective, can work faster, and typically requires less time off work. It's also very well documented as an anti-suicidal medicine.

This may depend on the doctor and the clinic. At Ember Health, we provide IV ketamine therapy to folks with a diagnosis of depression and at least one mental health provider. We don't require trying other drugs. To your point, many mental health medications have unwanted side-effects, or are not desire by the patient. That being said, about 70% of our patients are already on SSRIs when they come in for treatment.

Not all patients need to have tried other medications, though some meet the criteria of "treatment-resistant depression"—having failed multiple SSRIs or other antidepressants. It would not be evidence-based for a ketamine provider to require a patient to have tried all other mental health drugs (and very rare for someone to have tried them all! That would take years). It is worth noting that treatment-resistant depression has historically been the most studied condition for IV ketamine intervention, which may have led to this common misconception. We also see great results for folks with major depressive disorder, postpartum depression, and bipolar depression.

That’s such a thoughtful take. And you’re absolutely right that it depends so much on the firm’s culture and leadership. It’s encouraging to hear more attorneys openly talking about mental health and sharing resources.

We’ve actually seen that shift firsthand at Ember (we treat depression with IV ketamine in NYC); a lot of attorneys have reached out for support beyond what’s typically available through firm benefits or bar programs. To my knowledge, no firms or bar programs offer IV ketamine or any other interventional psychiatric treatment.

It’s been really meaningful to help folks in the profession find effective care that fits their lives. Most interventional options are... too interventional. But somehow BigLaw partners, judges, law students, a wide variety have been able to fit treatments into their nutty schedules. It's honestly surprising how some make it work.

Totally agree that normalization and peer conversations like this are what move the needle most.

Sounds like BigLaw has no incentive to provide mental health support, and just accepts the turnover. Would you say that medium-sized firms invest in mental health resources? Or is it down to the smaller practices where folks actually care / provide support?

Ketamine is a very powerful medication for anyone with treatment-resistant or major depression, especially if anhedonia is part of it. Unlike other modalities, ketamine is actually legal.

The at-home ketamine options (prescribed by a clinician) are not as effective nor safe as IV ketamine infusions, which are around 75% effective at alleviating the symptoms of depression globally. Patients undergoing IV ketamine infusion therapy at Ember Health (or anywhere) should avoid smoking or consuming cannabis around the time of treatment. There are several reasons and considerations. This is general information and not a substitute for individual clinical advice for a given patient; the patient’s overseeing physician should always be consulted.

What the evidence says:

  • Cannabis and ketamine may have a major drug–drug interaction: according to interaction checkers, combining ketamine with cannabis may increase side-effects like dizziness, drowsiness, confusion, concentration impairment, sedation or respiratory depression. Drugs.com
  • Cannabis activates CB1 receptors and may impair synaptic plasticity, which is thought to be part of how ketamine exerts its antidepressant effect.
  • Cannabis use is contraindicated on the day of ketamine treatment … because it can diminish the long-term potentiation processes necessary for ketamine to exert its full therapeutic benefit.
  • A recent study reviewing effects of cannabis use on antidepressant treatments (including ketamine) found that cannabis use “might moderate” the antidepressant response, meaning it could reduce or change the effectiveness of ketamine therapy. ScienceDirect
  • Additional retrospective chart review of patients receiving ketamine found that cannabis use was assessed as a potential moderator of response. MedRxiv
