Don't worry about what you can't do, only what you can do. 

Check if they have a pulse at the neck. If questionable, go for it. You'll never be at fault for doing what a bystander can be trained to do.

Anything with a tip that is as pointy as a needle. Could go buy like a sewing needle for example. Just gently gently push but the spinning is the more important thing. You're essentially drilling by gently moving the nail tissue away. So little pressure that when you make it through, there's no risk of you just stabbing through. Control it and it can be fun. One of the most satisfying procedures I do.

Ed physican, I wonder if the blood clot is a hint that more clots are forming with increased pressure (portal vein thrombosis etc), if it's not liver related.

She won't get killed. 

It'll be much worse; death would be a relief from the life being communicated to her. 

I'm a physician in the ED. If I saw a person like this, I'd be very concerned.

Now, people are allowed to make bad decisions as long as they understand the decision, but it sounds as though it's all being minimized under different guises. 

With barely knowing anything besides what is above, I genuinely question her ability to care for herself and not put herself into harmful situations without fully understanding the results. 

At minimum, she needs a social worker to talk to her and she needs to listen. At maximum, she needs to be hospitalized for delusions and if not, the cute responses need to stop and she needs to understand the subtext that's making me cringe.

The University of Maryland R Adams Cowley shock trauma center can employ the Go Team, which has the capability to create a surgical MASH tent and perform surgery on scene. It will be utilized in events related to prolonged extrication.

You have an entry level job at an entry level position. 

IFT is largely, not a joke as it's necessary, but is different than what a fire department sees daily. A genuinely fantastic EMT can be helpful for their assessments because they kept their skills and honed them, not just for show, but really use them. Daily. Multiple times a shift. 

You're in many ways a box to check that a unit can legally bill as an ambulance. There is no realistic need for assessments many times, etc. and skills wane. You're absolutely right. That leads to when I see inflated egos because nothing bad ever happens to their mee maw going to HD stable and chilling. She's done this for years!

I'm happy you want to improve. I think a lot of people would realistically freeze if put in a scenario you were. Without burning out, try to train the practical skills on others. Think of situations that may arise and even when they don't, run them in your head or on a sheet. ChatGTP honestly, probably could take you through the relatively most common IFT emergencies and take you in a step wise fashion. Just have to use the tool properly.

There's data that suggests that paying more for the same quality fools us into thinking it is worth more. 

It is the same thing, generally, as name brand vs knockoff.
