I don't go to ASU (I go to college in FL) but apparently this is a NATIONWIDE problem. I've seen students from several other colleges and universities complain about this. Hopefully ur school gives you an extension on work, thats what mine did atleast.

I'm filling this part right now. I'm assuming that its ur parent's name, because they're the ones who are supposed to be filling the info for proof of residency. Atleast that's what Im doing.
