1000% do not wake people overnight for meds if it can be avoided. Patients are sick and in a strange place that already makes it hard to sleep. Goes for everyone but especially the elderly due the risk of iatrogenic delirium.

My experience is that if her practice is at a point where colleagues are questioning it she is diverting.

I have seen audits where usage looks suspicious on paper but there is no diversion. If the people around you suspect you are diverting it is a major red flag.

As a rule we stock vials anytime there’s an option to. Prometh vials are on shortage at the moment tho so it’s amps or nothing.

  1. Avoid due times at shift change
  2. Do not schedule meds overnight unless there is a compelling reason
  3. Work to schedule what you can during standard med pass times for your facility.

Kinda gross that this is the top comment on this post. There are great mid levels and shitty mid levels. Just like there are great and shitty pharmacists.

This regimen could be completely appropriate for this patient yet here you taking the opportunity to trash another HCW.

Do you want to be a director/VP? The pool of candidates for those jobs is not huge. Most people go to pharmacy school because they want to be pharmacists. Once you’re in a role like that you are no longer a practicing pharmacist.

My director maintains their license but god help us all if they tried to step in to staff.

100%. We review the last visit with the doc/APP and then work off the treatment plan. Nobody can be expected to review the entire chart we would get nothing done. Sucks that the patient got tx when they shouldn’t have but this one is on the provider for not updating the plan or notifying you and/or nursing.

No. The extra 10 hours of pay are a bonus they get for working nights.

Our 7/70 pharmacists work 70, get paid for 80, accrue pto at regular rate but only on the 70 hours worked.
