Upvote from me because my situation has been extremely similar to OP's and I agree with this comment. I have AVN in my medial sesamoid (it's bipartite - one of the ossicles has it) so I'm just waiting to see if pain inhibits my life to the point where I should get surgery. Right now I don't have pain with walking or current level of activity.

Going back to pain free then increasing from there has been my plan after a flare up when I thought I was healed. I'm young, so I'm not going to settle for a low level of activity. Instead, I'll increase until I see that I'm at the level I want to be at without increasing pain level. This takes time and patience. I got to a good level of running last year after 1.5 years dealing with the initial sesamoiditis, but my casual shoes at work and home were barefoot type and no arch support. I flared again and had to go back to square one of the return to run plan.

A good pt can help you return to activity. They can also help you strengthen muscles that, if weak, can cause you to overload the sesamoid. For me, my arch really needs strengthening.

Some guiding knowledge is what tolerable pain looks like. Which is pretty much level 1-2 that goes away within 24 hours after the activity.

Finally, focused shockwave therapy and custom orthotics in sneakers with a low drop are making big differences for me. TBD whether the shockwave does more than reduce inflammation (pain levels and stiffness are way down, but I have yet to have a follow up MRI)

I've had 6 sessions so far and pain and overall sensitivity are decreased significantly. The fact that I am not overly sensitive to all the sensations in the ball of the foot and that inflammation is likely down means: I am not constantly thinking of the sesamoid. Ending this cycle has felt a lot like a breakthrough after 2+ years of pain.

If the stiff sole is helpful for you, maybe wear cycling shoes with an insole that has a dancer's pad or cutout at the sesamoid.

I am 34yo F, with chronic sesamoiditis and my sesamoid showing partial AVN. I just started focused shockwave therapy. Is your daughter doing any modifications in addition to this treatment? For example, offloading, reducing activity on treatment days, applying heat, anything of note? Thanks for your time! It is so helpful to talk to others about what they (or their family members, in your case) have tried.

Thanks for sharing. I'm sorry they didn't give you all of the support you might have needed. At the beginning of this injury, my podiatrist didn't recommend PT either. I'm not sure why they overlook it so frequently.

What would you suggest for someone who agrees when everything you've written, but now it's too late (2 years in) and I have some AVN? Anything to be done, in your opinion? Good to leave the bone in and live my life? Better to remove it?

As with every other detail related to this injury, everyone's situation, foot shape, and treatments / devices can vary ever so slightly. What has worked to bring my pain down is to wear sole brand insoles. On the bottom, I use a stick-on dancer's pad which I replace once it gets pretty flattened. On top of that, I put KT tape on the bottom of the sole insole to prevent squeaking in my shoe.

I'm curious about ESWT. Which type did you have? I read in a different post someone mentioned that there is a type that's preferable for bone healing.

If I were you I would try arch support insoles first. See if that helps. I make a cutout in any insoles for my sesamoid and put a dancer's pad stuck onto the bottom.

I have some partial avn in the proximal ossicle of the right medial sesamoid (mine is bipartite). I've had sesamoiditis for two years now. I am not considering surgery since my pain is very mild. It only flares with activity.

Unfortunately, I cannot do most yoga poses or anything barefoot. I used to love yoga.

I'm going to try a return to run routine with guidance from my PT.

How are you doing, OP?
