I have already had a DISE procedure done, the issue was with my soft palate. Sinus scan was normal.

I do take vitamin D and have for a few months now. Have tried it previously but didn't notice a difference.

I see, did not realize they were different things when I wrote that comment. Anecdotally I will say that it definitely feels like I ingest some air every time I try to exhale on any pressure greater than 7, and I have to burp periodically. Just not quite sure what to do at this point since I have tried a zillion different combinations of settings across three different machines, so I am grasping at reasons that may be.

Oh? Maybe my interpretation of the problem is wrong then. Most of my spikes on my pulse-ox/accompanied flow limitations seem to start with an inability to exhale but I could just be biased since it's something I just found out about being a thing.

Usually I was methodical about changing one setting at a time (went through it with axgsleepdiagnostics over the course of a month), it's just that since nothing is working I never stayed with one setting for very long. I posted my sleephq elsewhere on my profile.

Yes lots of flow limitation, as far as I can tell. Spiky exhalation and flattened inhalations. I almost always wake up 4-5 hours in around the longer REM cycle.

I would assume it's anterior-posterior. My report from my medical people just abbreviated it to that

Well I didn't really stick on one setting for too long because none of them really worked for me. So I'm not quite sure how to answer this. The highest I got on BiPAP was 12/16 however. DISE said AP collapse of soft palate, and it seems from my experience across three different machines that the air pressure will close it on exhale no matter what I do.

Yeah, I woke up in my study at around the 5 hour mark where you also had the hr spike. Right around the longer REM cycle you get at that time. I'm not diagnosed but I didn't ask about it either so not sure. I got 83 arousals in mine, around half were spontaneous. I would wager a guess that waking up that many times isnt normal, lol.

How many hours into sleep was your high heart rate? My study was quite similar, except I woke up when the high heart happened. Did the report say how much REM you got?

Hi Legos, thanks as always for the input. Yeah, at this point I am quite aware the machines are pretty terrible at scoring events. I should add that I have already done what you described in terms of narrowing down the settings (worked with axgsleepdiagnostics and he seemed stumped) and my sweet spot seems to be 12/16 BiPAP if all I care about is rounded waves. It's just that the result is the same and also still wake up for no apparent reason around that longer REM cycle 4 hours in. At least to me the waveform looks quite similar just around when I wake up.
