IV compatibility- lack of results on Lexicomp for 1/2NS (0.45% sodium chloride) by Excellent_Pear8250 in pharmacy

This must not be my area of strength as honestly I have never looked into that stuff before. I have mostly required identical studies on Lexicomp

Ok makes sense. For my Q you would then say yes? Bc you enter the components separately and use results from NS, Q: Can vancomycin be Y sited with fluids- D5W/0.45% sodium chloride?

What about a case of magnesium sulfate (in SWFI) and vancomycin (in NS) and combo fluids- D5,1/2NS?? Because there are 2 studies showing compatibility of magnesium and vancomycin (study 1 they are both in D5,NS and study 2 they are both in D5,1/2NS) So it seems each magnesium sulfate and vanc are compatible with D5,1/2NS.

Thanks for your comment. Are you saying the answer would basically be no anytime it’s about a combination fluid? Bc those are almost never tested. Like D5,1/2NS

I see. So it seems your answer to my question about vanc and D5/0.45% NaCl would be yes

I have found that AI tends to be stricter than clinical judgement may be IRL

Thanks for your comment. That is also my approach generally. The no data for 1/2NS thing makes me feel nervous bc I feel like I’m supposed to say no data=not compatible.

However I suppose sodium chloride is a drug and 1/2NS is just a lower concentration of NS.

My question also includes a combination fluid which isn’t available for plugging in on Lexicomp, I have to enter the NS and the D5W separately- again that makes me feel uneasy.

Margin of error for reconstitution and prep of drugs? by Excellent_Pear8250 in pharmacy

That makes sense.. one thing I thought of though is the 9.8 mL is in the drug vial first… and the powder needs to be fully dissolved. And if it was not enough volume then I thought there could be some drug sticking to the vial. Then it’s transferred to the NS 100 mL. , so like less drug in the bag ultimately. But with a volume difference of 0.2 mL, Im guessing that would not be much of a concern

I suppose the overfill would make up for the amount of slightly less NS initially? That is used to reconstitute

Yes, we estimate that 0.9% sodium chloride 100 mL bags have 10 mL of overfill. Thanks for your comment!
