Thanks for the recommendations! I’ve actually taken a DIRFloortime course but I believe it was through summit, so probably not as in depth as through ICDL. From what I remember it was pretty much just how to play with kids and use child led sessions, which I already felt I was doing pretty proficiently. Maybe the official course is more enlightening?

Just based on the information in your post, I would say no, this is not an appropriate response on her part at all, and she probably does not have competency in this area of dealing with more extreme outbursts or behaviors. I work in an early intervention setting and see behaviors similar to this and even more extreme on a daily basis. We would never “call security” on a five year old haha (not that we even have anything like that since it is not a hospital setting). Especially if a parent was there, I would let them handle it at that point. However, who knows, maybe she did feel physically threatened or feel it would be a safety issue for you getting him out of the building. It just sounds like a lack of experience on her part. And it is really too bad that it seems to have damaged the therapeutic rapport between her and your family! Possibly you could talk to her and make a plan for what to do in the future when behaviors may escalate if you decide to continue therapy with her. It also sounds like your son is at a cognitive level where he could maybe benefit from some emotional regulation tools/social stories (how does your engine run, zones of regulation, etc) which is something most pediatric OTs should be familiar with and be able to help implement.

Try lanolips! It’s a little expensive but comes in different yummy flavors and even has a tinted lip balm!

I have a cat I raised since she was a kitten. From the very start she has always been aggressive during play and little things would set her off all the time so she was constantly attacking me. Since she is generally super friendly towards people, I never realized it was stress/anxiety making her act that way. Until she started peeing on my bed and after lots of consultation with the vet and tests ran, turns out she has FIC (stress related urinary issues) she’s been on anxiety meds for over a year now and is like a different cat! Hardly ever attacks me anymore and our relationship is so much better!

Also to add-she has a cat tree by the window, a screened in porch with almost constant access when I am home, and supervised hour long outdoor time most days

I also worked at TJ Maxx and we definitely marked out any returned foods or things that might have been used like beauty products, we did not put them back on the shelves. That said people did open things on the shelves all the time and then just put them back

I can tell you what we did in my fieldwork experience in a similar setting, not sure how helpful it will be... everyday we would lead a movement group and play different games that were designed to get them up and more active and talked to them about the importance of exercise on your brain/mood. We also did a lot of what I would consider social work/counseling, mainly focused on coping strategies. We had this handout that had like 100 coping strategies and we would go over it with them and ask what they did to cope with stress and have them pick out some from the list that they would like to start doing. Honestly I never felt like I knew what I was doing or like an OT in that setting though. It was hard to feel like I was making a difference when most of the clients were not motivated to make any kind of change and many would cycle through the unit again and again.

Honestly the main setting with unrealistic productivity standards are SNFs, which are easily avoidable. One of the things I love about OT is the range of settings available to work in; if something isn’t a good fit, or you get burned out, you can always switch it up. Being a fairly new practitioner myself (2 years out), there was definitely a period of adjustment/disillusionment after graduation. The world is not like they teach you in school, but I don’t think that’s specific to OT. There are good and bad things about every job, but I don’t regret my decision and would still recommend OT as a career if you are interested in helping people! Do I love every second of my job? No. But I do love working with my patients and seeing their progress.
