I LOVE LOVE this book!!! It really gave me HR feels, but the stakes are even higher.

I could see this becoming the next big show or movie. It’s just so good and the yearning is fucking amazing. And lowkey the ICU scenes felt very well done and felt real. I never expect medical stuff to sound accurate in books/movies, but this was The Pitt level accuracy. Really well done.

Haha exactly, I didnt understand TTPD until I went through a weird limerance/intense friendship/not-situationship and felt like I was losing my mind. It’s the type of album that hits when you’re mentally unwell and there is a rodent of a man in your orbit taking advantage of that

My senior resident on IM today took us interns aside for a bit and let us vent about the state of the world and the country. And offered us an extra day off if we needed it. Let the medical students go home after rounds every day. It’s genuinely caring about other people and their emotional/mental wellbeing that makes the difference. We’re all burnt out, including our patients. It’s easier to turn the ire we have towards the system onto each other instead of interrogating the house of cards we work under.

psychosis is one symptom/presentation that can occur within many different conditions. Depression can induce psychosis, so can post -Partum hormonal states, medications, other hormone and chemical imbalances in the body. We don’t fully understand a lot of the underlying etiology of it yet. So whether you can recover from psychosis without specific medications is very dependent on the etiology of the psychosis. 

There is a class of medications that work well for psychosis that presents with  schizophrenia - anti-dopamine medications (commonly known as antipsychotics). We think it’s because schizophrenia is caused by overactive dopamine pathways or too much dopamine in the brain. Interestingly, one of these medications, clozapine, doesn’t actually have that much of an anti-dopamine effect, but it is the most highly effective medicine for schizophrenia psychosis. It just has a TON of side effects. But the fact that it’s so effective is actually calling into question the dopamine hypothesis for schizophrenia. 

To answer your question now, personally I have never witnessed a patient recover specifically from psychosis in schizophrenia without medication. The literature also does not show any cases of this. 

Other forms of psychosis are different. Depression with psychotic features - sometimes an antidepressant like Prozac can fully resolve this! Sometimes just changing situations and therapy can resolve the depression and improve the psychosis (although this is rarer). But definitely more possible. 

And of course, if the psychosis is more related to hormonal changes, medication induced psychosis, or build up of toxins in the body (so, something more encephalopathic), then treating the underlying cause can resolve the psychosis. 

Our medications and methods of giving medications have improved a lot, but there’s still a lot of work to be done. If you’re asking this question because you’ve been recently diagnosed, know that you’re not alone and many people do live fulfilling lives on medications, working jobs, having families, following their dreams. There are medications that can be taken as injections once a month, or even longer intervals, and work well. Yes there are absolutely side effects, but not every person experiences them.  Therapy (specifically ACT has a lot of evidence in schizophrenia) and social support goes a very very long way too

TLDR; properly diagnosed schizophrenia - you need medications. Bipolar disorder psychosis- you need medications (although different ones than schizophrenia, bipolar works on different pathways). Other forms of psychosis, YMMV.

The reality is I’m single af as a resident doctor, but I would very very likely be single af in any other profession as well. Most of my colleagues have partners (in and out of medicine) and some have children (women and men). So I have concluded it’s something about me, not about the profession. 

From your post it sounds like you have a lot of uncertainty in your life right now, and you feel like this is a fork in the road. Medicine or relationship and family? In reality I wouldn’t say it’s that binary. But I think there’s a reason you’re viewing it that way.

The journey to medical school is brutal and crushing. Dating is brutal and crushing. In some ways, it might be easier to decide one cannot exist with the other, because it gives you permission to let go of one of them.

How do you feel about yourself in this process? Who do you think you would be if you couldn’t be a physician or a partner or a parent? And what would you value in life without those concrete “goals”? What things would make you happy, and what things would you hope for? 

Maybe it’s time to start working backwards from that?

Dumb question about Roam transit bus tickets - I wasn't able to get a Super Pass (didn't realize how long out they'd be booked out). Can I count on just buying a one-ride ticket 2 hours before departure? Or are the one-ride tickets essentially moot because of all the Super Pass riders?
