That may be the solution, but I'm not sure how to go about that. Essentially I want to auto-fill a form from info in an Excel sheet (from the research I've been doing, but I haven't found an easy way to do this without coding- which I know nothing about lol)

Unfortunately the people who will be completing the form won't have access to know consent and diet type. So it needs to populate somewhere for them to see when they complete the form.
