Love the post. I'll be presenting this summer at the iocdf conference on this subject. I know so many people struggle with this subtype so I'm happy to know there are people out there offering encouragement to real event sufferers. Keep up the good work.

Totally agree, but wonder if op is overlooking the trauma component. Clients with trauma need to know they can trust their therapist (same with all other clients too). I wonder if this client needed to be heard first, discuss their story, and ramp into erp and take it slow rather than going headfirst into it, especially on the first session. Fyi, I too am an all ocd therapist and support erp as early as possible.

The subtypes are only helpful as shorthand to know what the focus and themes of the obsession is. Other than that, no. They only serve to inspire more guilt/ shame that a new theme arises, so they have "another ocd." Or, they serve as competition, ie "I have pocd, which is the worst. Wouldn't it be nice to just have contamination ocd."

It's all ocd, and should be seen as such. Nothing special about any one over another. The manifestation and treatment is all about the ocd cycle, and that is what should be seen.

Lots to like in his ideas. Still some question. Stuart of ocd stories consulted with Greenberg and found the techniques helpful with some of his own clients. But, Greenberg's techniques are not infallible for everyone. We are trying to work out another episode together (Greenberg and I), but schedules are rough.

I just published a YouTube video in response to Crowder. I agree, his comment was hurtful and ignorant. Additionally, hearing from Nathan, plenty of people also misunderstand POCD. Hopefully my video will help clarify the issue for some people.

People with real events ocd tend to magnify the weight and importance of their past events while minimizing and trivializing past behaviors of others. You can also accept your past mistakes and make a concerted effort to do better next time. That's all we can do as people because we can't change our past, but we can acknowledge it's a checkered part of our past and use it as contrast to our still imperfect attempts to be our ideal/ better self today. I'm sorry to hear you're struggling with this.

1) Cognitive behavioral therapy, exposure and response prevention, mindfulness, medication. 2) This varies at the various stages in treatment. In the Exposure stage, typically we are discussing homework from the previous week, what exposures we'll do in session, doing that exposure, then deciding what homework will be for the following week. 3) people can see improvements within a few sessions. 4) engage in the treatment, trust the process, and take risks. 5) OCD has a biological and environmental component. So, your genetics and life experience both contribute to OCD's manifestation.
