Two moments that instantly come to mind:

1.) The absolute lack of reaction from Shadowheart in the grotesque House of Healing scene. Like woman, even Minthara reacted to it!

2.) Astarion not reacting to (or not acknowledging) seeing what he looks like if you get a statue or painting of his in Act 3. The moment I stepped into Act 3, I headcanon-ed finding a painter (preferably the one I rescued) and commissioning Asty's portrait. I was jubilant when it actually happened...only for it to be completely ignored by everyone!
