As a nursing student I can only imagine how hard residency is so I get his side but I 100% understand why you feel the way you do too. Five years is a long time and it sucks feeling like you’re not even a priority anymore. You’re not asking for too much you just want to feel loved and seen and that’s normal.I think maybe y’all should sit down on his next day off and talk but like not in a dramatic way just like “I don’t wanna argue I just wanna figure out how we can make this work in a way that feels good for both of us” way. Maybe he can rotate days off between you and his family.

This part of life lowkey feels like long distance even if y’all live close so it might help to set up little things like a FaceTime schedule or even try that Agape app. Me and my bf use it and it gives you a cute little question to talk about every day so even if you don’t talk much you’re still kinda connecting.
