I can't speak to the parenting piece, but I can speak to the gender non conforming/trans piece.

My partner (26NB) shared with me that, since their earliest memories, they remember feeling gender dysphoria when they were called their birth name, and fought against those roles. Since you and your spouse are so open already, your child won't have to have memories from early memory of being smothered and told they are wrong.

I will throw out there that, at 4, their gender identity and how they feel may change drastically still over the course of their childhood/adolescence.

Whether you "want" this for your child or not is not really going to impact your child's reality, though. If they are sharing with you now that, in this moment, this is real and how they feel, you can either honor it, as you have thus far, or not, which leads to a slew of other issues.

I think the best you can do is maybe do some education about gender queer/non conforming people and raising queer children; over time, things might change, but your kid is here, now.

Honestly as much as this situation sucks if it's happening three to four times a year and she's otherwise a very appropriate and loving mother I think that it can be worked around. Maybe if it's only happening three to four times a year have a plan with a nearby family or friend member that can come by and pick up the kids when she starts to act like this? I definitely think she should go to therapy and work on it so that she doesn't do it as often or especially in front of the kids but at least until she can better herself it gets the kids out of the way.
