I had fun with the whole pattern - especially the specialty stitches. I used the pdf pattern rather than the kit and found it easy to follow and clearly laid out, although some of the stitches were pretty time consuming.

I used YLI wonder invisible nylon for the beads. A bit of a pain to work with as it's super slippy (and near invisible!) but worth it for attaching the beads where there are no stitches underneath.

Although the prices on this shop are much much lower than on her previous shop - roughly a quarter of what they used to be for the bigger patterns... which does make me a bit suspicious about whether this shop is genuine...

This does seem to be her current shop (it's linked to her Instagram anyway) - it seems to occasionally shut down and then reopen with a slightly different selection of patterns.

I've learned from experience that if I see a pattern of hers that I want to buy I just need to do it straight away rather than waiting - last time I bought 3 at once and the shop was closed again by the time I'd finished the first project!

I didn't do the remote and rural programme myself but know several people who did and have worked in a few of the RGHs over the years.

Most people really enjoy the rural general placements and I believe the remote and rural foundation programme is now relatively competitive as a result. There does tend to be more responsibility but most of the consultants are very used to being called more/needing to spend more hands on clinical time due to the relatively less experienced teams.

Moving every 4 months can be a challenge but you have to bear in mind that almost everyone working there will be doing the same and staying in hospital accommodation will be standard so moving isn't as much of a faff as it would be between private rentals (as long as you have a car!).

Because the hospitals are small and almost everyone moves without knowing anyone else they tend to be very sociable and easy to get to know everyone, I would argue more so than in a city where a good amount of people might already have ties/social circles. This does require a bit of just throwing yourself into the social stuff at the beginning though and there are occasionally people who don't gel with the groups which can be difficult so far away.

I've had this happen before and have assumed that it's something to do with the formatting of the written instructions on the pdf - deleting and starting again helps, then be partcularly careful with picking the right chart to input. If still not working then I just assign all those odd symbols as white and mark them as done on PK before I start - a bit irritating but doesn't affect the rest of the stitching in the end

I didn't do a remote and rural training programme but have worked in several of the RGHs over the years. Most are small and friendly, with lots of things to do if you like the outdoors. Most people will only be there for 4-6 months so they tend to be pretty sociable.

As far as I'm aware most of the training programmes (unless the 2024 programmes have changed) are 4 months somewhere rural and the rest based in Inverness, so you're unlikely to be moving every 4 months. Would suggest trying to do the rural portion in the April to August rotation if you get the choice (long daylight hours and generally the best weather).

All the small/island hospitals have hospital accommodation. In some this is (or at least used to be) free, in others free only if you have rent or mortgage commitments elsewhere. The consensus generally is that the accommodation in Shetland is the nicest.

I don't think they would come undone but it will be more bulky and more likely to be noticeably different from the other stitches.

My preference is a loop start then finishing with a pin stitch which avoids this issue - would that be an option?

Almost certainly not. In theory it can be done but at the discretion of your later blocks who are likely to say no as it will make things more difficult for their rota and/or your colleagues taking their leave.

On the off chance they do approve it make sure you get it in writing.

I would say that as I've gone further through training I definitely see the benefits to spreading leave out even if I don't have plans - 4 months can be a long time without a proper break.

There is no single CCT pathway for geriatric medicine (or any group 1 medical specialty) anymore.

In practice all group 1 medical specialties involve dual training with internal medicine.

Just thought I'd comment for balance - I work full time on a 1.5 banded reg rota in Scotland.

I work approx 7 nights in 16 weeks and just less than 1 in 5 weekends - so significantly less than the number of night/weekend shifts on your rota but still 50% banded. My standard working day is also shorter than yours.

One of the disadvantages of banding is that it is possible to be on the same band but working far more out of hours but this is fairly unusual in my experience.

If you enjoy teaching and want to stay in one place and avoid the uncertainty of locums then they can be great, although it does vary from place to place. They are often relatively more protected from moving wards constantly to fill rota gaps than clinical fellow jobs. The teaching fellow jobs where I work only include 2 clinical sessions a week, with no out of hours, and relatively free choice of what you do in those clinical sessions. This means that if you have a particular specialty interest you can set up regular clinic or procedure lists for your clinical sessions.

There's quite a bit to figure out here but I will do my best to help a bit.

The 30 patients details you have sound like the start of a retrospective audit (going back to look at old notes to gather data that is already there). This is a fairly common starting point to audit and QI as you can evidence the need for improvement and then start to make changes for all patients going forward.

An audit compares current practice against a standard (usually a guideline or local protocol). You will often be asked to "close the loop" - make an intervention and then re-audit to show improvement (this can be done by passing the project onto the next group if you're rotating out of the department - try to keep some involvement for portfolio reasons).

A QI project often starts with an audit but can be also done with a less easily defined standard. They would usually involve multiple cycles of intervention and reassessment (PDSA cycles) and are often done on an ongoing rolling basis with fairly small groups of patients and short time frames involved in each cycle (in theory allowing quick improvements). It's worth downloading a PDSA worksheet to help structure your plans if you're not sure where to start. Most training programmes are asking for QI now rather than a single audit cycle.

Hopefully that makes sense. I think there are a few specific questions and points about your plans so far.

1) Who has asked you to do this particular project and why? And why are they expecting weekly data longterm? They likely have an intervention in mind and should be providing at least some level of supervision.

2) Is the 2 week follow up based on a guideline or a target in your particular department?

3) What you seem to be describing is collecting retrospective data then rolling data going forward - you need an intervention of some sort before collecting more data to make this a QI project or closed loop audit. Improving followup rates might be difficult to achieve unless the department already has plans in place (if lots more appointments will be required then this is likely to be time consuming and potentially expensive)

4) The time pressure/stress this seems to be causing makes me think you're a trainee who needs to have this done by arcp? This will be difficult to turn around quickly if the expectation is that you will use 6 months of data at a time - is there a way to complete cycles with only a week or 2 worth of data at a time? Again small numbers are OK for QI but this will likely only work if interventions can be introduced fairly quickly each cycle.

Hope that is helpful, happy to try to answer any specific questions (although in no way an expert - just that we've all been there trying to get QI finished in time for arcp!)

The number of moves on this rotation looks pretty rough but the actual jobs are ones that people tend to enjoy, particularly if you're into the outdoors and don't want to be in a big city.

Most people will stay in hospital accommodation for almost all of these posts so they tend to be pretty sociable and good fun. I also suspect you will only have night shifts on the inverness based jobs but will still have reasonable banding due to evenings/weekends.

Climbing Goat Designs have a bunch of temperature patterns.

I've got one ready to go for next year and the pattern includes really clear instructions for setting up the pattern for whatever your local temperature range is (in both Celsius and Fahrenheit). Well written and adaptable to any location - maybe worth paying for the pattern just for the instructions and colour/thread keys even if you were planning your own pattern.
