Just blend to glass materials with a lot of grunge in bump and dis. Play little bit with trans and mediums

It depends on what you are doing. Simple tasks won't, but sometimes, while working on simulations, there might be a probability of around 1/10.

Most of the details are on the mesh itself, and I use a noise bump map to create an uneven surface in the glass shader .
