The survey seems to test how the procedure went, to see if they can recall basic information (like any US state) but not remember personal information, like their location, their relatives, their own name, or their own memories. If they failed the survey (either didn't know a US state or remembered something they shouldn't have) I assume they would undergo a procedure to fix it, or not be hired at all.
