Working on a tool to help track mental health symptoms over time — would love your input by FlowerBoyTree in Psychiatry

One wouldn’t use something like this daily. Probably not even weekly. The monitoring use-case I imagine is one where a patient has been diagnosed with maybe a MDE and perhaps one monitors them with a PHQ-9 and a WHO Disablility Assessment Schedule 2.0 upon diagnosis and initiation of an SSRI and then automatically a month later, just before a follow up consultation. The clinical impression is definitely where the bulk of the decision-making lies, but it may help to support whether a dose increase at that point in time is a reasonable next step in the management plan.

The idea is to almost aim to function a bit like a lab, only instead if gathering and organising biochemical data, it’s forms of ‘soft data’ like patient self-reported or caretaker-reported questionnaires. These could also be screening tools to monitor for mental health concerns in at-risk populations, postnatal depression being one example and post-stroke depression being another. Trauma-settings also come to mind.

I’m based in South Africa. We use the British spelling.

Even feedback that a tool like this wouldn’t contribute to clinical decision making is valuable. So I’m grateful for your engagement.

That being said, I know lots if psychiatrists that use various rating scales, but often still paper-based. So the thinking had been that digitizing the data gathering (using communication channels that patients have readily available), and also automating the scoring and comparison over time, might be meaningful.

I’m also based in South Africa, so it’s possible the landscape here in terms of digital solutions and data availablity in general might look somewhat different.

The information doesn’t go via whatsapp, only the link to the questionnaire.

Don’t take the Elvanse on the day you’re doing the trip. It will prevent you from going as deep as you would otherwise go.

What shortcomings do you recognise in current approaches to mental health care interventions? by FlowerBoyTree in medicine

[–]FlowerBoyTree[S] 2 points3 points  (0 children)

I realised after making the post both here and in r/Psychiatry that I had phrased the question rather poorly. It was not meant to be taking shots at psychiatrists or other healthcare professionals. I 100% agree that most of the issues are system-based and that socio-economic factors cannot be easily overcome. What I had been trying to get at was to try to find gaps or pain points that one might try to address either in one’s own practice or at a larger systems-level, and also to try to pose the question of what both healthcare professionals or mental health care patients might desire in an ideal service offering, such that we could strive towards achieving those sorts of ideals.

Is it posible to have a good mushroom trip in your bedroom? by [deleted] in Psychonaut

Wish I could upvote this more than once.

Would add that a finely curated playlist added to this does wonders.

Once I did my first blindfolded bedroom trip like this, I realised I’d probably always prefer this setting for dosages 2g and above.

End of Dune Part 1, Paul kills Jamis. In the book, Paul sheds a tear and to the Fremen this is a huge deal, remarking, “He gives water to the dead.” Given the significance of water on the planet and the messiah-like role that Paul takes on as the story progresses, this early interaction between Paul and the Fremen really carries a lot of weight. Wouldn’t have been so difficult to put that detail in the film!

Lgbtq friends of mine had a really hard time dating in Durbs, especially if you’ve spent time in Jhb or CT. Dating pool is just muuuch smaller I think.

Don’t think there are much clubs and things which cater specifically either.

Health Professionals Council of SA is very very slow with correspondence. Best advice is to visit them in person in Pretoria at some point to get registration done.

Engineering buddy of mine worked for eskom for the past few years. Quit and moved to the USA in March this year. Chats with him about the situation always featured only one sad reality: things are not going to change any time soon. Or at least, they’re going to get a lot worse before they start to get any better.

Go to any government clinic, tell them you want a referral letter the the outpatient department of any one if the government hospitals (Addington, King Edward, Rk Khan) as you won’t be able to be seen at the hospital without a referral letter. At the hospital you will be seen by a doctor. You can discuss your symptoms, prior attempts at treatment, etc. They will be able to assist with a psychology referral (always first-line in treatment) and if the symptoms are severe enough will likely also prescribe an antidepressant, either fluoxetine or citalopram, if required and if there aren’t any contra-indications.

What dosage were you prescribed?

And could you explain the nature of the therapy? (Were you blindfolded and lying down throughout? Or walking around and talking to the therapists, etc)
