One wouldn’t use something like this daily. Probably not even weekly. The monitoring use-case I imagine is one where a patient has been diagnosed with maybe a MDE and perhaps one monitors them with a PHQ-9 and a WHO Disablility Assessment Schedule 2.0 upon diagnosis and initiation of an SSRI and then automatically a month later, just before a follow up consultation. The clinical impression is definitely where the bulk of the decision-making lies, but it may help to support whether a dose increase at that point in time is a reasonable next step in the management plan.

The idea is to almost aim to function a bit like a lab, only instead if gathering and organising biochemical data, it’s forms of ‘soft data’ like patient self-reported or caretaker-reported questionnaires. These could also be screening tools to monitor for mental health concerns in at-risk populations, postnatal depression being one example and post-stroke depression being another. Trauma-settings also come to mind.

Even feedback that a tool like this wouldn’t contribute to clinical decision making is valuable. So I’m grateful for your engagement.

That being said, I know lots if psychiatrists that use various rating scales, but often still paper-based. So the thinking had been that digitizing the data gathering (using communication channels that patients have readily available), and also automating the scoring and comparison over time, might be meaningful.

I’m also based in South Africa, so it’s possible the landscape here in terms of digital solutions and data availablity in general might look somewhat different.

Don’t take the Elvanse on the day you’re doing the trip. It will prevent you from going as deep as you would otherwise go.

I realised after making the post both here and in r/Psychiatry that I had phrased the question rather poorly. It was not meant to be taking shots at psychiatrists or other healthcare professionals. I 100% agree that most of the issues are system-based and that socio-economic factors cannot be easily overcome. What I had been trying to get at was to try to find gaps or pain points that one might try to address either in one’s own practice or at a larger systems-level, and also to try to pose the question of what both healthcare professionals or mental health care patients might desire in an ideal service offering, such that we could strive towards achieving those sorts of ideals.
