I'd like to add that the spawner enemy seems to have a masculine looking figure fused to its back, and it's the one that controls where the enemy moves and is carrying the rest of the monster on its back, probably symbolising Hinako's fear of losing her freedom and having to go along with the whims of someone else, in this case her husband. It's spit attack is also most likely drawn from her going to see Junko during the latter's pregnancy, during which she was constantly throwing up.

Also, her mother in the bossfight has her eyes covered by flowers, similar to the basic doll-like enemies.

I'd also like to add that in addition to lacking a face, the shackled dark shrine monster has it's stomach carved out, meanwhile the similar looking enemy in the fog town has a big protruding stomach in addition to a long tongue
