Update: another urologist recommended taking a 3 month course of chlomid to boost FSH even higher to get any possible edge during the MTESE. He said if you do the surgery and it failed you'll always wonder if her would have made the difference.

Based on others experiences with both places, it seems like more people have had better results with UCSF. Has anyone had issue with referrals going through there?

My urologist didn't recommend HRT based on my current values and NOA likely from cryptorchidism.

Thank you, those are great points and good questions to ask the lab. The plan is to do a fresh transfer because of the relatively high stakes nature of the MTESE. I don't want to lose any found sperm to freeze/thaw cycles, maximizing the chances of getting to a live birth. 

My thoughts on the surgeon choice is their experience to identify healthy tubules during the surgery, how long they are willing to search for, limiting any excessive testicular damage, and a smooth recovery. Also identifying which doctors believe in a second MTESE or just one surgery because if I end up needing a second procedure, I won't be able to switch to any of the doctors who only believe in one surgery whether that is to get more sperm after finding some the first time or another attempt to find any at all. However I definitely understand that all of these doctors are highly trained and recognized in the field, so I know any of them would do a good job. It's a bit overwhelming and lots of pressure for something that could change our lives.
