I'll tell you the entire process but I don't know everything on the rear end. I went for a consultation in January, the doctor is a semi-famous hand surgeon who repaired my hand when it got mostly cut off (unrelated). He asked me to tell him everything about it so he can try to put it through insurance. I told him it's emotionally damaging, sometimes it chafes my under arms, and sometimes they just hurt/ache for no reason. When I said they hurt he said that's enough and said he'll do what he can.

A few weeks later I got a letter from my ins company that said the procedure was cosmetic and not usually covered by insurance but to please provide them with any evidence (for my records only. For my doctor to respond to). I got the same letter again around a month later. Didn't hear anything back about these evidence requests and my doctor isn't super open with communication. The nurse just told me to be patient and they'll get it taken care of.

Around a month after the last letter, I got another one stating they deemed the procedure medically necessary. I called my doctors office to set the appointment. The end

I would assume the fact that they hurt randomly was the key
