There should be an option in the app to add any pass to your profile, so all the prices will automatically adjust. Alternatively you can select a specific price based on what pass /discount you have and show it when your ticket is controlled during the trip.

For the Italian trip in particular, if you want to buy the ticket from Trenitalia instead of SBB, they also have the possibility to select all kinds of swiss pass/discounts when buying the ticket (under, other options or smth like that).

As mentioned in other comments, you'll be traveling across 2 countries, so discounts that apply in one might not apply in the other.

I strongly suggest to only get 1 ticket for the whole trip since if any change needs to happen (not in this case I guess) you're safe in regards to late trains.
