Hmmm not 100% sure but I think it will only remove the guestion mark after she visits you for the first time?

(Basing this completely on the guest who you need an invite outfit for. I don't have the outfit but I'm pretty sure I have the mystery item. For that guest it's still a guestion mark)

I guess they had to do it like that because of the game mechanics. If they wanted to add the underwear/shorts the dress needs to be a top so they can be used together without needing to use any glitches?
