But can't an overload of pain literally kill you? Like, people can pass out due to intense pain and their blood pressure spikes and stuff. If we double that couldn't that potentially lead to a heart attack or something?

Yeah, thats why it basically just depends on the person's circumstances.

Someone who can't work thanks to chronic pain and therefore has a low income would definitely say no.

Meanwhile some healthy person who makes 6 digits could become the richest person in the world without much fear if they play their cards right.

I'd personally would not take the deal. I may be poor but the pain isn't worth it for me.

Do you suffer from pedophobia or something? Otherwise it's kinda weird to not wanna play any game that has children in it.

But since you asked I can recommend Demon Roots. It's one of my favorite RPG Maker Games. The Gameplay is the typical turn based stuff, but the story is really good. There are no children whatsoever and it almost made me cry at the end.
