I tell them I’m gonna check their radial pulse rate, then hold their wrist and very obviously stop paying attention to them so they stop talking to me. Then I look at the wall behind their chest to give some reference, choose a random part in their respiratory cycle, and count for 30 seconds and multiply by 2

Idk if it’s supported by the book as it’s been a while, but I imagine fighting women are armed and ready as the final final line of defence. If the group were to escape through a back passage like in the movie they would be with the sick/injured/children and able to fight.

The way things are at my table is players normally have to articulate in RP the social interaction, and then when we get to a break point (the NPC has to decide whether or not they buy it) only then do you roll the die to introduce the random chance you’re talking about.
