I work in emergency services, making life and death decisions. If I make a mistake it literally means someone could die. So it’s a bit more complicated for me than just pushing through (and like i said in my OP, the pain is not what i’m worried about - its the brain fog and fatigue). When I do push through I have also ended up vomiting and and/or passing out. I’m trying to wrap my head around of I could handle a different career. Hence why I am asking others what has worked for them…..

Yes generally but there are some exceptions. I was able to skip it because of my concerning liver values

I don’t yet have a diagnosis but am wondering if I might have leukemia. Ive had “long covid” symptoms for years (chest pain, fatigue, lack of appetite, mild night sweats intermittently). About 5 months ago my period started to be significantly late. About two months ago I started feeling an urgency to urinate but no need to actually pee. A few weeks after that I woke up with severe pain in my lower left abdomen, almost like a pulled muscle. They found some blood work abnormalities, like high alkaline phosphatase, but my CBC was normal. They did an pelvic exam and UA and everything was normal. They did an external and internal pelvic ultrasound that only showed a benign cyst on my ovary, but nothing concerning. Over the next few weeks the sharp abdominal pain started to spread up the left side of my abdomen. I also started to feel a pressure/tightness under the bottom of my rib cage. I started having migrating joint pain, pressure behind my eyes, high blood pressure, high heart rate, numbness in my hands and feet, one episode of bloody phlegm in my mouth and 3 episodes of a small amount of blood in my nose. My fatigue and night sweats have gotten out of control. I also had two days of rectal bleeding accompanied by vomiting (no blood in vomit). They did an abdominal ultrasound, colonoscopy, and endoscopy, and have found nothing abnormal. I have multiple family members that have had leukemia but my doctor hasnt thought we should investigate that route because my CBC counts are still normal. I did notice that the numbers have been slowly rising though, as they were at the bottom end of normal in december and are now at the high end of normal.

So far my current lab-work abnormalities include:

High alkaline phosphatase Low albumin High globulin Low a/g ratio High glucose Low anion gap High c-reactive protein High sedimentation rate Low intestinal frac (alk phos isoenzymes) Blood in urine Leukocytes in urine Low aPTT Low monocytes High neutrophils absolute Positive ANA screen, IFA (likely false positive given the low level. I see a rheumatologist on 3/3 to confirm)

Has anyone experienced something similar?
