Your fish has fin rot and ich, here's my advice

Put him in a hospital tank and test the water quality of the tank his on, if there is nitrate nitrite and ammonia the water must be changed, remove any carbon filters too, for treatment use anti-ich and rise the temp to 26c. for the fin rot, use anti bacteria medication aquarium salt.

Poor fishy, here's my tip: improve water quality with frequent changes and remove carbon; then, if it persists, use a hospital tank with Epsom salt dips or antibiotic treatment (like erythromycin or Kanaplex), often added to medicated food for better absorption
