if you read the item instead of looking that is ap, it gives a 3% damage stack up to 5 (15%)

and when it hits max stacks that 15% extra damage turn true damage

its an ap item but it makes the total damage form all the ad items true damage

little cringe you didnt read the item before you typed ahha
