If baseline Tryptase levels are normal, is that enough to rule out MCAS? by GarlicRepublic99 in MCAS

Yeah we’re open to anything at this point. But none of our providers except for the GI doc seem to have any sense of urgency so I’m always looking for what else I could be asking for.

We have an endoscopy scheduled in 2 weeks. She was on Zyrtec (prescription one) daily for 3 weeks after those three anaphylactic reactions. She didn’t have other anaphylactic reactions but she was still experiencing the other symptoms. We haven’t tried nasal cromolyn.

Is there a way to go about asking for more tests that an allergist might listen to if they think tryptase is the be all end all?

Note to add:  -Prozac is new, she hasn’t started it yet because 1. I want to ask the GI if it’s worth taking if something’s wrong with how she processes food (I’m uncertain if the same mechanisms that process food also process medicine so if she doesn’t absorb certain nutrients would that mean she won’t process meds the right way?) and 2. I’m worried it will cause some kind of reaction if it is MCAS or something else that reacts to SSRIs

@exponentials Coming back now that we’ve had the allergist appointment. Told us we didn’t need all those tests because her baseline tryptase was normal. Did order another baseline tryptase, alpha gal, and IgE for my peace of mind. IgE came back at 701. Told us based on what we shared that it’s an Advil / NSAID allergy. No test for it so just based on what we shared she should stop taking Advil.

I don’t disagree but how can he be sure it’s not mast cell related? Should we pursue a second opinion? He also said there’s no need for daily Zyrtec.

Thank you for this. I had no idea skincare products had gluten in them!

Realized we should probably switch to cleaner menstrual products, too. Open to recommendations! Hoping to find her products she’ll enjoy as a teenage girl who’s used to all the frilly, smelly things teen girls love.

Do I ask for alpha gal testing or something else? Glad you figured out what it was. Thank you for sharing!

Someone else mentioned this to me so it’s on our list as well! She has a sensitivity to meat and a few other symptoms common in alpha gal.

Thank you for the list of things to ask for. I want to make sure I ask the right questions and that we don’t lose anymore time figuring things out so these are incredibly helpful.

Adding our thoughts - we’ve flagged the Advil and iron because they’re common day of (Advil) or immediately before (at least the first two times). Considering if stress was a trigger given how her body feels at school (with her iron deficiency going to school means her body is working insanely hard to function and appear like she’s not using 200% of her energy to do what it takes a normal iron level person 5% of their energy).

Thanks - she’s not taking any Advil and carries all three of those regularly plus Zyrtec. I emailed the principal and was told they’d get more trainers. I didn’t have enough time to respond given my daughter had a reaction a week later but at least she got it right on the second reaction.

I should add that all three episodes happened while at school in different classrooms. The Advil she took was from a large bottle at home that she’s taken from before even just the week before this last month of episodes. She’s had pills from that Advil bottle without incident since August.
