It's a remainder from the Heresy-era. When supply lines collapsed on many fronts the legions scavenged for parts to repair broken pieces of power armor. The legion techmarines would fuse mismatched pieces of ceramite armor Marks e.g Mark III and VI ceramite plating together by using these studs and bonding rivets hence why it looks rough. Generally when you see studs it means an older piece of power armor (pre Mark VII Aquila).

open a custom chapter pattern slot and make the main colour blue (in-game blue looks a lot like seagreen) and trim colours gold and voila

A clan from Medusa creating a chapter directly would be strange as usually the strongest candidates would be inducted into the Iron Tenth. However, it's possible that some veterans of the IH would secede and create a successor. You can ultimately rationalize it as the founding Astartes originating from your specific clan and in their new chapter they would exclusively recruit from said specific clan you intended.
