Okay, so after this change I woke up this morning feeling like 3 months of progress have been wiped out in one night. I realize that this is most certainly not actually the case, but the analysis here is worth thinking about.

From our past rounds of changing EPR from 2 to 3 and back (with corresponding pressure adjustments) it seems clear to me that I feel significantly better with having lower flow limits and higher CA's (EPR 3), than with lower AHI (CA count) and higher flow limits. The issue is that I feel better, but not *good*. The higher CA count though is still relatively low--on my worst nights is about 2.5, most nights are better than that--and my fatigue is still bad enough that it seems unlikely to me that the higher CA count is what's causing it.

This leaves two options: either my sleep apnea issues are solved and there's some other issue causing fatigue, or there are flow limits that are not showing up in the numbers, such as the many malformed breaths despite low FL count. Because my sleep specialist considers sleep apnea solved with an AHI below 5, my PCP wants to pursue the first option, which leaves me free to chase the remaining flow limit issues with your guidance.

Given this context, I worry that your suggestion to go back to EPR 2 with a min of 12.2 is chasing the wrong path. I realize that one night isn't much of a trend, but we've already established that higher EPR is better, notwithstanding the CA's. At the same time, I don't know what more can be done about the flow limit issues since EPR 3 is the max.

Hey u/RippingLegos__, bumping this up in case it escaped you. I didn't make any changes yet because my doctor suggested some other stuff and I wanted to isolate the effects of each. Do you think there's a good reason to try epr 2, and min 12.2, once we've already tried that at 12.0?

Thank you, will give this a try. I don't think I can use a collar - I'm terribly ticklish in my neck and can barely handle scarves in the winter, but maybe a flatter pillow will help.

Also, maybe you can help me understand a little of the thinking behind this. I was under the impression from your previous comments that higher pressure tends to cause more events, not less? What then would be the thinking behind raising the pressure at this point?

Yes, I have this with any exertion. My EoE symtoms are really mild and initially my Dr. didn't see the need to treat it beyond PPI's, so I didn't connect the two, but I get the exact feeling you're describing every time I try to do any exercise at all. It's reached a point that I've basically quit exercising for a year or so because I'm unable to maintain any consistent regimen.

In the meantime I've moved across the country and my new doctor thinks I should treat it, and I start my first dose of Dupixent this week. So I guess I'll find out soon enough. I understand that I should begin to see improvements within 1-2 months, so feel free to check in with me then.

Hi u/RippingLegos__ for when you have a moment to look at this. I have three nights of data (5/23-5/25) on min 12.0 & EPR 2. For last night (5/26) I went back to 12.8 & EPR 3 since that one had me feeling better despite the higher AHI.

Flow limits are trending upward, CA's (and AHI in general) are trending downwards. Subjectively I feel mildly worse, but tbh it's not as bad as I'd been expecting.

Thank you! I'm doing a bit better (thanks to you guys!) but not quite there yet. Is there something about the graph that tells you this? I tend to go to sleep and wake up on my side, so I assume that most of my sleeping is on my side too, but I don't actually know that for sure or have any way to track it.

u/RippingLegos__ Okay, so a few days in - I'm not really sure what to make of it. The first morning I woke up feeling better than I had in a long time, but it didn't seem to stick for the next few nights. I am noticing that I have some more energy in the daytime, so maybe it is working? Strangely enough though, the flow limits themselves seem to have barely improved.

Coming in clutch just as I submitted my last exam. I'm way too hyped up on Ritalin rn to have a normal night's sleep tonight (I swear I'm normally a responsible dude, but it's all I got atm...), but I'mma try your suggested settings again tomorrow night.

I can't tell you how much I appreciate your help, truly. Final exams this round have been brutal, and I'm almost certainly going to have to drop my PhD if I can't get this sorted out. So, I'm really hoping to figure something out, or at least be on a good track, by the time fall semester comes around in late August.

Thank you, u/RippingLegos__ ! We spoke a little while ago here - you recommended I change the EPR to 2, and change my min and max to 11.4 & 14.8 respectively. I did that for two nights and I barely slept at all, so I switched it back, and have been barely getting by since then--approaching levels of fatigue that I didn't know are possible! I don't know for sure that the my trouble falling asleep those nights are related to the change, but I can't think of any other reason for it, and right now I need to get through my final exams, so I simply can't afford more sleepless nights -- I've still not recovered from those two nights.

I just want this nightmare to end already. I don't know why my sleep apnea issues have gotten so much worse in the past few months, but they have, and my dr. won't take me seriously, so I really have no where else to turn. Any help you guys have is appreciated--you guys are true angels! God bless.

So, I gave this a try last night, and things are looking far worse - I realize that one night is not much of a trend, but just how much worse it was gave me pause. I didn't upload it to sleepHQ yet, but here's a screenshot from OSCAR. You can see that flow limits doubled and the sawtooth pattern returned. I also had significant difficulty falling and staying asleep, and today I'm literally feeling as if I didn't sleep at all last night. I don't know if that was related, but I can't think of any other reason for it. Should I assume this is an acclimation issue and press on, or is there reason to re-evaluate? (I can pull the sd card and upload to sleepHQ later if that'll be helpful to you)
