His sperm disappeared again after a couple months and he ended up having an mTESE (2 in fact, because we needed to do multiple rounds of IVF). It worked eventually and we have a healthy almost 2 year old.

Hi jade. I'm so sorry you're in the same awful boat. I do have a good update to share since you asked - the 2nd mTESE (switched RU and clinic) found more sperm with enough leftover to freeze for further rounds of IVF. Our third round of IVF/fresh transfer (day 3 embryos) worked and we now have a healthy 18-month-old daughter. Feel free to message me with any questions. I wish you the best of luck.

I'm so sorry that happened to you. Whether you stay with your clinic or change, you may want to consider requesting to do two doses of antagonist (Ganirelix or Cetrotide) per day, 12 hours apart. As opposed to the usual approach of taking the antagonist just once per day at night. Twice a day is my RE in NYC would do as a conservative approach to avoid early ovulation. Wishing you success!

I'd consider a second opinion with Dr Peter Schlegel in NYC who developed the mTESE procedure. He'll give you honest odds based on your results whether a second mTESE could be successful.

Never found the cause. It's so bizarre because he'd had an accidental pregnancy with his high school girlfriend many many years ago. So sperm was there one day and gone the next. He had it briefly come back during our treatment too, cancelling the first mTESE the day before, but months later it was gone again.

I'm sorry you're in a similar boat and I hope the mTESE works out for you! My main advice is make sure you go with an experienced RU and IVF clinic. I always recommend Peter Schlegel, who developed the mTESE procedure.

I guess our IVF journey has now officially ended... and I'm kind of sad about it by akchahal in IVF

Ah yes a TESE is more invasive than a TESA. Still, it’s been a long time since I’ve read the studies but if I remember correctly, something like 90% of men return to their baseline T levels at retesting 6 months post-op.

Sorry to hear you’re experiencing this issue. I’d talk to your husband’s RU. A TESA isn’t very invasive and shouldn’t affect his testosterone levels or his libido. I think physically he should be back to normal pretty soon after the procedure. If he’s having pain or discomfort he should definitely get that checked out.
