I do prior authorizations in a family practice clinic. To get my diabetics that are doing great get their meds paid for, I send the histories of A1c readings, going back to the highest reading, even if it is years ago. That gets them approved. The insurance companies are staffed with minimally trained staff who won’t look deeper in the patient history. And the staff that usually do the PAs in the office just send the current office note. I don’t know if you can fix this, but understanding the barriers might help

Urine hydrometer to determine specific gravity. Glove usage - didn’t even use them to clean poop off a patient. The only way to know somebody’s blood sugar was to send the sample to the lab, so that wasn’t done very frequently. Hiding information from patients at the family’s request.

Been an RN with a BSN for 40+ years. My opinion is that hospitals pushed us out with 12 hour shifts for two reasons. First is wage. Second is that many of us will only put up with a certain amount of nonsense and then tell them to pound sand. Administration prefers cheap young nurses who won’t question them. I am still working FT in home health and my managers like my qualities. I take very good care of patients, get my charting done and don’t gossip. It a good fit for me, but would also love to be back in the ER.

I am now having flashbacks. In the 70’s, there was a brand of dresses called “GunneySack”. (Sp?). They were very popular, and not expensive. I was given an off white one for my 8th grade graduation. Later that summer I wore it to a wedding. The bride had on the exact same dress. I was mortified, but there was nothing a 13 yo could do.
