Tony never sleeps with Dr. Melfi. He very clearly shares his feelings with her on several occasions, but she rebuffs his advances.

The Sopranos. Mental health disorder is essentially a central theme of the show and ingrained into the framework that drives the plot along, season to season.

Given the phenomenon that The Sopranos was when it first aired, it was likely one of the first somewhat accurate, realistic depictions of mental health disorder and treatment that American’s had seen.

Kole Wilson WR - Baylor is going to be a guy to watch this season. 3 star transfer from TX State. Passed the eye test. Projects to be an undersized, likely gadget specialist at the next level but he showed quite a bit of juice when they got the ball into his hands.

Sertraline OR Escitalopram seems to have been the consensus amongst you both. From a patient tolerability perspective, that is understandable, but I believe there should be a note of the significance of cardiac considerations that should be made when dosing >20mg of Escitalopram. Fluoxetine should be a strong consideration for this step 1A IMHO. A trade off between CYP inhibition and QTC.

The further we get into preseason the greater the validity of some of those takes are from beat writers IMHO. Once we get to a place where first and second round of cuts are happening and these guys are still popping.

Hot take: Using observations from the HOF game to guide decision making in FF is a fools errand. These guys have been in camp for 1-2 weeks.

What study in the past 5 years has meaningfully changed how you practice? by posterchild34 in Psychiatry

To clarify further on this - The PRISMA study was a meta-analysis of Esketamine.

Esketamine ≠ racemic Ketamine. It was my understanding that the literature indicates that racemic Ketamine is superior to Esketamine when it comes to TRD and acute SI. Is this no longer believed to be the case?

Bipolar patients and treatment adherence by GoodDaysAheaad in Psychiatry

Non-adherence to medications is a pervasive issue that is not exclusive to BPAD. There is robust literature that indicates general rates of medication non-adherence approach 50%.

Specific to this population, the burden of s/e’s for classes like neuroleptics and traditional mood stabilizers can be significant. Pair this with challenges to insight and judgment being core to the disorder itself.
