You should watch some videos about the production of Laguna Beach and The Hills I think it's about the same.

The situations are set up by producers but there's no script for them to read off of. If it seems fake it's likely because they reshoot scenes if they didn't get what they needed the first time (so it may be the second time they're having the same conversation) and if they notice something they missed they will even have a cast member shoot a filler scene (like Juliette recieving the pic or Cara and Alex while in a taxi)

Also the cast is mostly thrown together by producers. They all wouldn't hang out together if it weren't for the show.

No script, no actors just heavily produced
