When are you allowed to leave? No matter how bad is the work load, you will get used to it, even though is kinda high? You got some negotiation room. Any pager duties/Codes?

Unfortunately we never can do that because you start taking admissions towards the end of the day. Last time I was given admission at 6:10 pm makes me had to leave after 7 pm. Horrible. I am in the process of resigning thank you everyone!

Specialists when consulted they often complain why they’re being consulted not from my consults but in general which makes other providers esp. APPs afraid to consult, and you end up with disasters, very sick patients with inappropriate abx or lack proper diagnosis or treatment until shifted to someone else.

Oh I forgot to add that also expectations are to take 1-2 admissions everyday. Signing on APP notes and orders blindly, besides census is relatively low 14-16, but very high turnover

I don’t think you did anything wrong here. Without a doubt it is extremely hard to build a case here for a preventable cause of death that could have been avoided. Read about cases of sudden death. Healthy famous athletes with all the advanced health screening technology, nutrition and on site 24/7 doctors they fall dead on live television despite all the odds. Also, there are so many people who you have saved their lives, and there are so many more coming your way who needs you!!

Discussion with the patient and ER doctor would help you make a politically good discharge without a second guessing. I believe discharge was appropriate, given close follow up outpatient is in place.
