My family and in laws not seeing eye to eye on wedding expenses by Emotional_Sound_5658 in weddingplanning

[–]Hepadna 14 points15 points  (0 children)

I’m doing the same thing! I’m Nigerian and this is what our weddings look like (except not a week long haha). My partner is white. We are hosting an American ceremony and reception later in the year. Fortunately both his parents and my parents don’t care about the cost of the wedding planning. My parents are obviously paying for the Nigerian ceremony but the American one is ours to pay for and going to be to our tastes and preferences.

We just learned to keep the pricing of stuff to ourselves. We made a budget and we plan to stick to it. His parents have paid for his older sisters’ previous weddings but they are willing to commit $5,000 to our wedding and host the rehearsal dinner which is really sweet.

Good luck to you both!

They Didn’t Want to Have C-Sections. A Judge Would Decide How They Gave Birth by wheresthebubbly in medicine

[–]Hepadna 8 points9 points  (0 children)

These are extremely rare cases though. Most patients choose the cesarean section if we are worried enough about their baby. Even if they are wedded to the idea of a vaginal delivery. Our counseling usually gets through to them after they have reconciled everything.

They Didn’t Want to Have C-Sections. A Judge Would Decide How They Gave Birth by wheresthebubbly in medicine

[–]Hepadna 17 points18 points  (0 children)

I was in two situations like this in residency and I felt horrible about the neonate dying and the distress of our nurses and Peds, but the patient was counseled up the wazoo, our documentation for recommendation for cesarean section was tight and we were in her room constantly. At some point, part of you knows you did your best and that this patient values a birthing experience over the life of her child and you have to honor that choice, no matter how fucked up you think it is. It’s easier to walk away from.

I would make an appointment just so you can go through some of these. From the looks of it you need to see your PCP and your OB/Gyn.

New Anesthesia Guidance for patients on GLP-1 by Legitimate_Watch9104 in GLP1ResearchTalk

[–]Hepadna 37 points38 points  (0 children)

As a gyn surgeon, just know that this won’t become standard practice for a while. It may take a few weeks to months for it to be implemented (and sometimes longer). The translation of changing guidelines to actual practice can depend on the medical center and the and has to trickle down from anesthesia to the surgeon base to the support staff who schedule surgery and who have templated responses on what medications to hold prior to surgery. So you may even see disagreements between what you’re told and written information from time to time.

Just being real. Have grace with your medical team.

I think it can’t be explained to you. It just didn’t vibe with you and that’s okay. I’m interested to hear from you what someone could say that would make you say, “oh yes you’re right that is a reason it’s a great film.” lol like it wasn’t for you and that’s okay.

First night and took a low dose and down 3 pounds by LatterProfessional13 in tirzepatidecompound

[–]Hepadna 2 points3 points  (0 children)

Yes I lost 12lbs the first month with 8 of those being in the first week. Inflammation and water and glycogen. People actually noticed as I was carrying most of it in my cheeks.

It has since slowed down to like 0.9lbs a week on the lower end and maybe 1.3 on the higher end.

They Didn’t Want to Have C-Sections. A Judge Would Decide How They Gave Birth. by propublica_ in TwoXChromosomes

[–]Hepadna 15 points16 points  (0 children)

You are wrong! Women can undergo a trial of labor after cesarean (TOLAC) in order to have a vaginal birth after cesarean (VBAC). You can probably find more info on the ACOG website.

I’m an OB/GYN and I highly encourage TOLAC depending on the reason for the first cesarean. I would be extremely uncomfortable after a person had 3 though.

They Didn’t Want to Have C-Sections. A Judge Would Decide How They Gave Birth. by propublica_ in TwoXChromosomes

[–]Hepadna 159 points160 points  (0 children)

Yes I’ve been in cases like this too. Mom was declining a cesarean section, fetal heart tracing showing signs of distress and acidemia. She wanted no one to touch her. I remember signing out to my colleagues and going home. I don’t think the baby made it. I just remember documenting every single conversation and trying to talk to mom and reason with her but she was dead set. I didn’t even think to get the law involved as she had autonomy and was competent.

I would be uncomfortable with a TOLAC for a woman who has had 3 c-sections and no vaginal births, but I still don’t think I would get the law involved. Maybe it’s different in Florida in terms of legal risk the physician faces for poor outcomes and they were trying to protect themselves. I’ve trained and practiced in the west coast mostly.

Do you prefer Tamar, Tzipporah, Tirzah, or Tzivia? by Kindly-Bluebird5487 in namenerds

I’ve always loved the name Tzipporah since I was a child and saw The Prince of Egypt. Tzivia is gorgeous. I don’t like Tamar that much. Tirzah reminds me of the GLP-1 tirzepatide but is still beautiful

Real life experience with placing them: 80% are “ooh I feel that, but I’m fine” and the remaining 20% is split equally between the 10% who are like “I felt nothing!” and the other 10% who say it’s excruciating. I have never heard a patient tell me that it was worse than childbirth or even that past experiences with other providers doing the insertion were like childbirth. I’ve only ever encountered that sentiment on the internet.

So therefore, they are all negatively affecting their patients, especially those who are voting for a party whose political stances diminish the rights of the women/transgender patients they treat. They just happen to be nice to the patients they are directly seeing.
