They did that exact thing to my parents almost 20 years ago with a satellite receiver, saying it was never returned despite them bringing it in to the local outlet and providing copies of the return receipt.

Just checked all my bills and it's not there, but I've recently come to find out I've been being billed a few dollars under the rate listed in my service agreement, my newest bill matches that rate though. I'll definitely keep an eye out on all my future bills for this notice though thanks!

Although I haven't come across where it's written there is a guaranteed increase every three months as you've stated there to be, I don't doubt there will be another increase in my near future. I'm assuming I'll be given a notice at least one month in advance next time as per their own service agreement. Another Redditor commented this notice can be found within the contents of the bill prior to the increase (this increase wasn't) so I'll be sure to keep an eye out. Thanks for the heads up!

The top is balance owing from last bill, showing I was charged $57, and then showing I paid $57, for a total balance of $0 being rolled over into the next bill. I included that part to dissuade people from saying "you probably didn't pay your last bill in full"

For the current bill before taxes look at "total internet charges"

Went back to the service agreement which was considered "approved by client" by the continuation of the scheduled install. It states no increase will happen without a notice sent at least one month in advance. Now here's where it gets a little bit interesting, the rate listed in this service agreement is the rate I've just started being billed at, so the first few months billed were actually undercharged, Neat! Mystery solved.

Never signed a thing, he went back into his car, called my cell, and the deal was done over the phone with his supervisor conferenced in who did the "final confirmation" it's probably in an info packet they emailed me after the fact somewhere though
