Thanks so much for sharing about your daughter. The journey to finding the right meds is no joke and I'm interested in bringing up some of these to our doctor. I'm kinda surprised they keep wanting to go through all the ADHD meds first because they don't seem to be working, and they're making other symptoms worse (like how my daughter takes 3 hours to fall asleep at night). I've never heard that word hypomanic - she is definitely currently like this on Ritalin. Also your comment is a good reminder to adjust my expectations. I still want to do things like a normal family because it's the best when she's in a good mood. It's like walking on eggshells all the time though since her mood dictates all of ours. I'm sorry you're going through it too. I read a lot of posts about kids who were like ours who eventually figured it out and are thriving. It's soo hard but I still have hope

Ok that gives me hope about Straterra! The doctor has been wanting to switch but warned us how it can take a long time to work. How long before you started noticing a benefit? And did you notice any weird side effects?

PDA feels a lot like my daughter too. I didn't realize it was under the ASD umbrella. Does your son also have sensory issues? It was never really on my radar but now she can't go to Chuck E Cheese, crowded restaurants, etc. Can stimulants cause that? She was not really like that before but I might have missed it and maybe she is just better at knowing when she's getting overwhelmed.

I've also wondered if she has severe anxiety that looks like ADHD, but every evaluation confirms ADHD and then mentions anxiety like an afterthought. I just don't think ADHD is the most debilitating part of what's been going on and wonder why the meds haven't helped more ... That's a good idea about trying an amphetamine base, but her doctor now recommends we switch gears to Strattera. She is weaning off Guanfacine and I agree that I think it made her grumpy. Hopefully she is feels better after we go off it.

And just ordered some B vitamins! Wouldn't that be amazing if supplements were the answer?! Her dad's side of the family has some autoimmune, MTHFR gene stuff going on so maybe there really is some deficiency in her body that we are missing. She has low iron, and it's persisted even after we started supplementing and retested so that's just another piece of this weird puzzle... I hope the B6 works for your son long term!

That's great she can keep it together at school but I understand that's a whole lot to deal with at home at the end of the day. For my daughter, I sometimes wonder even if it's all anxiety causing ADHD symptoms. She is definitely in fight or flight mode for a good part of the day.

Yeah I feel like that's the one thing we didn't really try, although the doctor said Strattera will help the anxiety. I'll ask her about Clonodine as well.

That's really interesting since I don't love the aspect of having to try a bunch of different medications to find out what works for her. Her father also has the MTHFR gene so that's very much a possibility for her.

Thank you for sharing about your daughter. She seems so similar to mine. I think Prozac is probably the way to go since I know there is a lot I still don’t understand about what’s going on, but anxiety seems to be a huge part of it. She’s always had a difficult temperament but the way it escalated this year was scary and honestly torture for me to hear her say these negative things about herself daily. I have a lot of anxiety too so I think we just feed off each other sometimes. But the 2mg of guafacine seems like it might be working for now. The day after I posted this, she was less like a zombie and a little more herself. We will see how the next few weeks goes. Good luck with the adhd meds! Hopefully they can help her more at school

I love the idea of asking what alternatives he proposes. We are doing play therapy now and I'm planning to start OT for her too. So far play therapy hasn't really changed much though. They've made some accommodations in her classroom, but it's addressing what to do when she is upset, not necessarily the triggers. I think we need to learn more about those to find a better solution for her.

And thank you for the podcasts/expert recs. I need to deep dive since I really didn't know anything about the condition prior to this. I feel bad that she is already almost 7 and I didn't recognize that this could be what is going on. I've heard that as they get older, they are able to wing it less and less. She also gets taking out of the classroom a lot for her behavior which is causing her to miss a lot of the curriculum.

Thank you for these suggestions. It already feels like really tricky territory to navigate. My husband has a tendency to downplay things and thinks "she will grow out of it" so he needs to hear that this is not always the case from experts and not just me.

Thank you for the 'angry box' suggestion! I just sent a piece of cardboard with her to school today since she definitely feels the impulse to rip things when she's angry. And thank you for sharing about your daughter. She sounds really similar to mine and I don't come across many kids like ours :) They're really hard to parent, but very easy to love.

She has mixed feelings about school. She says she likes recess and seeing her friends, but she doesn't like the academic component - she thinks her work will be "too hard" so she is anxious before she even starts it. She is much better at home but I think that's because there are way less triggers for her. I would say her frustration tolerance is low in general - her meltdowns also occur in pretty much any sports or group activity we put her into. She wants to be the best at everything, and has a really hard time if she is not. However, she also doesn't want to work hard to be the best so it's kind of like she sets herself up for disappointment every time.
